چکیده

Pacemakers and implantable cardioverter-defibrillator (ICD) cardiac leads are used for delivery of short direct current electrical pulses. Low-energy pulses in pacemakers provoke myocardial stimulation while high-energy pulses in ICDs are delivered for lethal arrhythmia termination. Pacing impedance measurements offer insight into implanted pacemaker system status; a decrease in impedance values may indicate a short circuit or insulation breach, while an increase may indicate a pacing lead fracture. Remote interrogation systems that capture automatic device diagnostic data, stored episode electrograms, and the presenting rhythm provide the clinician with information similar to that available at an office visit, allowing assessment of the appropriateness of device therapies and operation. We report a series of 3 cases of transient high lead impedance (42500 ohms) on remote checks with no other associated lead abnormalities, and no clinical events or abnormality seen on chest radiograph or office checks. These cases shared a common type of device manufacturer with a different type of lead manufacturer. Although ventricular pacing impedance was sporadically highly abnormal, device function was not impaired, and system revision was not necessary.

Right Ventricular Pacing and Sensing Function in High Posterior Septal and Apical Lead Placement in Cardiac Resynchronization Therapy

BACKGROUND The conventional right ventricular (RV) lead position in cardiac resynchronization therapy pacemakers (CRT-P) is the RV apex (RV-A). Little is known about electrophysiological stability and associated complications of pacing leads in RV high posterior septal (RV-HS) position in CRT-P. METHODS Two hundred and thirty-five consecutive CRT-P patients were included from 1999-2010. Pacin...

Do daily threshold trend fluctuations of epicardial leads correlate with pacing and sensing characteristics in paediatric patients?

AIMS To evaluate whether the magnitude of daily ventricular pacing threshold fluctuations (Deltafluctuation) in trend graphs of stored diagrams correlate with ventricular threshold and sensing changes over time. METHODS AND RESULTS A total of 56 children received AutoCapture devices (St. Jude Medical, Sylmar, CA, USA) connected to steroid-eluting epicardial leads. Maximum lead age at study cl...

Do electrical stun guns (TASER-X26) affect the functional integrity of implantable pacemakers and defibrillators?

AIMS High voltage electric current can adversely affect pacemakers (PM) and implantable cardioverter-defibrillator (ICD). The standard shock from an electrical stun gun (TASER- X26, TASER International, Scottsdale, AZ) consists of a 5-s long application of high voltage, low current pulses at 19 pulses per second. Its effect on the functional integrity of PM and ICDs is unknown. METHODS AND RE...

A long-term, prospective, cohort study on the performance of right ventricular pacing leads: comparison of active-fixation with passive-fixation leads

Active-fixation pacing leads allow the use of selective pacing sites. We evaluated their long-term performance versus passive-fixation leads in 199 newly implanted patients (n = 100 active and n = 99 passive). Postoperative pacing thresholds in the active group were higher than in the passive group (0.85 ± 0.31 V vs. 0.53 ± 0.21 V at baseline, P < 0.001). The active thresholds fell to 0.72 ± 0....

Chronic Proximal Biceps Tendinitis in Young Patients with Anatomic Proximal Origin Variations: A case series

Purpose: To describe a case series of young adult patients with isolated chronic proximal biceps tendinitis refractory to conservative care found to have anatomic long head biceps tendon (LHBT) origin variations who underwent arthroscopic-assisted subpectoral biceps tenodesis. Methods: Patients were included in this retrospective case series if they met all the following criteria: 1) had ...
